Crash reconstruction underway after deadly Fairfax ambulance collision

Police say the ambulance had lights and siren on when a car struck it at a Herndon intersection.

FAIRFAX COUNTY, Va. — Fairfax County crash investigators on Tuesday opened a reconstruction of a fatal collision in Herndon after a car hit a county ambulance traveling through the intersection of Worldgate Drive and Alton Square a little after 4 p.m., killing a passenger and injuring three others, according to authorities.

The case is now with the Fairfax County Police Department’s Crash Reconstruction Unit, which handles serious and fatal crashes across Virginia’s most populous county. Investigators said they are analyzing signal data, skid patterns and vehicle damage to determine how the impact occurred while the ambulance was responding to a separate medical call under lights-and-siren. The driver of the civilian car and two Fairfax County Fire and Rescue firefighters were hospitalized. The passenger who died was an adult male, police said. Names had not been released as of late Tuesday pending family notifications and medical examiner review.

Police said preliminary information indicates the ambulance entered the intersection with emergency equipment activated when the car struck the unit’s side. The force of the impact scattered debris across multiple lanes on Worldgate Drive. Fire officials said the injured firefighters were conscious when transported and will be evaluated under department protocols. A statement from the fire department said crews on scene provided immediate medical aid to all involved before transferring care to hospital teams. Officers kept traffic blocked while detectives mapped the roadway with specialized equipment and documented evidence markers laid along the crosswalk and curb line.

The intersection sits between apartments, offices and retail near the Worldgate athletic club and is a busy cut-through toward Centreville Road, where evening rush hour can stack backups. Emergency vehicles in Virginia may proceed through red lights when responding, but must do so with due regard for safety. The county in recent years has installed more traffic-signal preemption units and trained crews on clearing intersections, though fatal collisions involving county apparatus are rare. Residents in the corridor said crashes occasionally occur near Alton Square during peak hours, especially when drivers turn left across through traffic.

Detectives said it is too soon to say whether speed, distraction or a signal violation contributed. Any potential charges will be determined in consultation with the Fairfax County Commonwealth’s Attorney after the reconstruction is complete. The Office of the Chief Medical Examiner will identify the deceased passenger and determine cause and manner of death. Officials said they intend to provide an update after gathering witness statements and reviewing nearby private cameras from businesses and apartment buildings along Worldgate Drive.

Neighbors watched from storefronts and sidewalks as wrecker crews loaded the damaged sedan and ambulance. A manager at a nearby shop said customers heard a “sharp bang” and saw smoke near the crosswalk as police arrived. Another passerby described firefighters working “quickly and quietly” to clear the roadway while tending to the injured. By early evening, most lanes had reopened, but cones and absorbent material remained on the asphalt while investigators finished measuring the scene under flashing lights.

As of Wednesday morning, police said the investigation remains active and that timeline updates will follow once reconstruction data are analyzed and next-of-kin notifications are complete. No court dates have been scheduled. Officials said the ambulance will undergo a mechanical inspection before returning to service, and an internal review of the response is expected in the coming days.
